PROCESS OVERVIEW

Our 3 step process provides your patients with complete care.
Step 1: Initial referral
Step 2: Sleep Study (w/ follow up)
Step 3: CPAP Titration (w/ follow up)

Our office will handle:
Contacting your patient, scheduling your patient, acquiring insurance precertification.

Quick turn around time:
Reports are sent to referring physician within 2 weeks of study.

If your patient requires follow up therapy: Our reports will come with a pre-filled "follow-up" prescription or "Continuation of Care" forms. (see below for examples) Sign and fax this back and we can expedite your patient's therapy.

CPAP:
Our office will find a Durable Medical Equipment company (DME) that accepts your patient's insurance. The DME company will visit your patient at his/her home and provide the CPAP machine.

CONTINUANCE OF CARE FORMS

Accompanying our reports are: Continuance of Care (COC) forms with suggested recommendations for continued treatment (CPAP Titration or MSLT test). Click forms below to enlarge.

OVERVIEW Our sleep-trained physicians will provide suggested recommendations. Our staff transcribes these recommendations and patient information on to COC forms. AFTER SLEEP STUDY: our staff will recommend either CPAP Titration or MSLT. Form will also include the Apnea/Hypopnea Index (AHI) from the Sleep Study. AFTER CPAP TITRATION STUDY: our staff will recommend CPAP pressure. Form will also include the AHI on recommended CPAP pressure, the mask-type used during the CPAP Study, and whether humidifier should be prescribed. SIMPLE AND EXPEDITED SERVICE COC Forms can be used as prescriptions for further treatment/therapy. Just fax MD-signed forms to our offices: (914) 333-5925.
